FSSA NOTICE Charity Cares Early Academy is a state-registered childcare ministry and a nationally accredited early childhood education program through NECPA. We exceed the required health and safety standards across our program, staff, and facilities, and we are fully insured to protect the children and families we serve. By signing this statement, you acknowledge and affirm the following: "I understand that this childcare ministry is not licensed under the laws of Indiana. However, I understand that this childcare ministry complies with the state rules concerning sanitation and fire safety for the primary use of the structure in which it is conducted. I understand that it is my responsibility to ensure that my child's nutritional and health needs are met while at the childcare ministry. I understand this notice does not absolve the childcare ministry from liability for injury to my child if such injury is due to negligence or intentional wrongdoing by the childcare ministry or its employees."
DISCIPLINE Charity's approach to discipline is guidance-focused, individualized, consistent, age-appropriate, and with goals for children to develop positive behavior and self-control. Discipline is never associated with food, rest, or illness. Staff are prohibited from shaming, humiliating, frightening, threatening, or negatively labeling any child. Staff may not inflict physical, sexual, emotional, or mental abuse. Corporal punishment-including hitting, shaking, biting, pinching, binding, tying, or confining a child to an enclosed space-is forbidden. Non-severe physical restraint may only be used when necessary to prevent harm. If a child's behavior becomes a safety risk or a family is unwilling to support behavior modification, services will be suspended or terminated.
FAMILY CONDUCT As an enrolled family, I agree to uphold and abide by the Family Conduct Policy, which applies to all my primary family members, parents/guardians, emergency contacts, and authorized pickups. Key expectations include: Respectful Communication, Appropriate Behavior, Drop Off & Pickup Compliance, Facility Access, Avoidance of Disturbances or Altercations, Appropriate Interactions, and the Strict Prohibition of Weapons and Drugs on our property. Any violation of this policy by any affiliated adult will result in immediate termination of my childcare services.
ACKNOWLEDGEMENT I acknowledge Charity's Open Door Policy. I acknowledge that my family will be photographed and recorded for safety, marketing, and educational documentation. I acknowledge the risk of group childcare settings, including common communicable diseases, routine incidents, and minor injuries. I agree not to pursue legal action beyond appropriate medical care.
PERMISSION I grant permission for emergency medical treatment and authorize the transport of my child during emergencies to and from school, on field trips, and for other program-related reasons. I permit my child to participate in on-site and off-site special activities and trips, including property owned and operated by Charity, community spaces, and outside venues. I confirm that I have fully read, clearly understood, and agree to comply with all policies and procedures outlined in the Charity Cares Early Academy Family Handbook and Family Contract.
