Purpose:
The purpose of this agreement is to specify the conditions of participating in respite services delivered through Autism Babysitters. When signing this agreement, both parties agree to abide by the terms and conditions as set out in this agreement and will cover all respite bookings made and delivered within the dates as specified in the agreement. Respite services are charged per the current year's listed price sheet.
Autism Babysitters aims to provide high-quality, customized respite services to every client. The client will be included in all choices regarding accommodation, meals, and activities.
Service Provider Responsibilities and Conditions
Work with the client to provide respite services that meet the client's needs and goals as described during consultations.
Discuss and plan the respite with client involvement, giving them choice and control.
Protect the client's privacy and confidentiality.
Always treat the client with courtesy and respect.
Consult the client if decisions need to be made about how the services are provided.
Listen to the client's feedback and resolve problems and concerns quickly.
Have insurance and necessary security checks to deliver services requested safely.
Keep clear and accurate records about the services provided to the client.
Issue invoices that explain what services are to be provided, their cost, and when payment is due.
Cancellations
Once respite services have been approved in writing, a deposit may be required of up to 50%. This may not be refundable. If cancellations or changes to bookings are requested, at any time after signing this agreement, the client will be charged (invoiced) to cover the hourly rate of the babysitter.
If the client cancels their respite at any time during the respite, for any reason other than injury or significant illness (requiring hospitalisation) that require the respite service to end, full charges for the entirety of the respite will b payable to Autism Babysitters. NOTE: This includes (but not limited to) any serious breaches as listed below in this document.
In signing this agreement, the client (or their Support Coordinator) confirms there is sufficient funding for the respite to proceed and the client is not experiencing any flu or COVID like symptoms.
Client Responsibilities
Pay the amount invoiced by Autism Babysitters within 3 days of receipt.
Ensure there are sufficient funds in your account to cover the cost of our services.
Provide feedback to Autism Babysitters to ensure needs are being met.
Always treat the Babysitter with courtesy and respect.
Discuss problems or concerns with Autism Babysitters to seek quick resolve.
Tell the Babysitter if there is a change to the Needs Agreement which is the basis for this agreement.
Tell Autism Babysitters if any contact details set out in this agreement change.
Give Autism Babysitters the notice required (see below) to end this agreement.
Changing This Agreement
If the Client and Autism Babysitters want to make any changes to this agreement, they must discuss the changes and mutually agree. The written changes should be signed and dated by the Client and Autism Babysitters before any new changes apply.
Ending This Agreement
If either the Client or Autism Babysitter want to end this agreement, both parties must give 2 weeks' notice to the other. A cancellation clause may apply regarding costs. If the Client or Autism Babysitter seriously breaches this agreement, the notice period will not be applied, and this agreement will end immediately, and cancellation fees will be applied and charged.
(Serious breach examples: abuse of any nature to Client or Babysitter, not having sufficient funds to cover the cost of respite service, unreasonable requests or demands that cannot be delivered resulting in conflict).
This Respite Service Agreement agreement is made between the Client and Autism Babysitters (Service Provider).
