AFC Stadium Information Form

  • 1. Introduction

  • Welcome to the AFC Stadium Information Form

    This Stadium Information Form captures key information relating to the stadium for the purpose of assessment in accordance with the AFC Stadium Regulations.

    Upon successful submission, an e-mail containing a link to download the AFC Full Stadium Information Form PDF file will be sent to the e-mail address provided in the form.

    The Form includes a Save and Continue function. Users are advised to save their progress regularly and to use a stable internet connection while completing the form.


    Upon selecting Submit, users are requested to remain on the submission page until the submission confirmation notification is displayed.

    Kindly ensure that all required information is duly completed prior to submission.


    Users are not required to register for an account or subscribe to any service in order to complete and submit the form. Simply click the Skip Create an Account button found in the bottom of the pop-up box.
