This is a new Approval and Workflow process for the 2026-2027 Season

This form is used to facilitate the submission & approval of the below requests. This form is typically filled out by an Association's Hockey Director or Coach with Hockey Director Approval-
- 8U to 10U play up requests and approvals
- 12U to 14U recreational play up requests and approvals
- Medical play down requests and approvals
- Out-of-State (Youth & Girls) player approval for participation in Colorado (USAH/CAHA sanctioned Hockey)

Medical Play Downs Require the following 3 Documents for review and approval-

1) Signed Letter from Medical Doctor recommending the player compete at a lower level.

2) Signed Letter in support from the Association's Hockey Director in support of the listed player competing at a lower level

3) Signed Letter from the player's parent's acknowledging the request for the player to play-down.
