Auction Item Donation Form

Thanks for your spook-tactular support of NEXT Village SF and its members through your donation!

NEXT Village is a 501(c)(3) non-profit corporation. Our Federal ID # is 80-0476158.
Your tax-deductible donation supports NEXT Village SF, a neighborhood non-proﬁt that helps older adults age in place and remain connected in their local community. In addition, it shows your support of seniors, or is a meaningful tribute in honor of a loved one.

You may use this form to make an Auction Item Donation for our online auction. Please complete a separate form for each item to be donated.

The auction kicks off October 1st and culminates at our Halloween party on October 26th, 2025. Auction items should be picked up or delivered to NEXT Village by October 10th. Please reach out to us to coordinate.

If you have questions, would prefer a hard copy donation form, or are interested in sponsorship opportunities, please contact NEXT Village SF at info@nextvillagesf.org or 415-888-2868. The Auction Committee will contact you if they have any questions.

We hope you'll join us at Halloween in the Village on October 26th in San Francisco's North Beach.

How will this item be given to NEXT Village SF?*
