Hello! First off, I want to start by saying THANK YOU so much for your interest in booking a tattoo with me.

Please read the following:

Before filling out this form, please make sure you have a generally specific idea of what you are looking to get tattooed, and that you have researched my work and are familiar with the style I tattoo in. I will use the information submitted in this form to design your tattoo.

All requests will be considered but not necessarily chosen. Please know, this does not mean your idea is not good. All requests WILL be replied to regardless. Booking information will be sent in the reply.

If you are wanting to submit more than one tattoo request, please fill out one form per tattoo.

If this is a request for matching tattoos for multiple persons, please have one designated contact person fill out one form for the tattoo.

PLEASE be aware that the ONLY email you will be contacted by is 'bookings@tatteredlacetattooco.com'. Scammers may try to contact you using a similar email address (watch for spelling/contact). All deposits are processed through Square. I will NEVER request an e-transfer to pay a deposit.

Please allow up to 4 weeks for a response. All submissions will be responded to. If your project is selected, you will be required to pay a non-refundable deposit to confirm your booking. If all policies are met, this deposit will be taken off the final cost of your tattoo at your appointment.*
