    Please confirm your booking details here. Pets collected before 10am will not be charged for that days stay. Pets collected after 10am will be charged for the full day. This information below must be the same as the information you provided to our clinic at time of your phone booking.
  • Your Pet Details

    If you have more than one pet staying with us, you need to complete individual and separate forms for each pet and each time they stay with us.
  • If you have listed your pet as being over the age of 8 years, we will need a pre-approval boarding form filled in by your current vet and provided to us.  

    The link where you accessed this form, also has an Older Pet Clearance Form which you can access.  Please download this form and give to your current vet.

    If your vet cannot provide a clearance form, please feel free to book a consult with us. Contact us to arrange.  

    Please note that we cannot accept your pet's boarding without this pre-approval.

    If you are a client of Plympton Vet Clinic, Marion Animal Hospital or Southern Animal Hospital our vets can provide this information - there is no need for you to provide this form as part of your boarding agreement as we can access your pet file.

  • To ensure we can provide the best quality care to your pet whilst they stay with us, it is important we are aware of their previous medical history. If your pet sees us at Plympton Vet Clinic or one of our sister clinics (Southern Animal Hospital or Marion Hospital) we can access this information internally and you can move on to the next question.

  • If you answered NO to the above question, you have indicated that your pet has not been seen by a vet within the past 6 months and does require medications with us during their stay.  All pets who require medications must have been seen by a vet within 6 months.  

    You will need to contact your regular vet or book a consult with us before proceeding further.    Do not continue filling in this form if your pet has not been seen by a vet in the past  6 months AND is on medications.

    • Pets boarding at Plympton Vet Clinic are provided the relevant 'Hills Science Diet' DRY FOOD ONLY (E.g puppy, adult, mature etc.) twice a day. 
    • Plympton Vet Clinic provides DRY FOOD ONLY. If you think your pet will require a wet food option, you must provide your own for the duration of their stay. Please return to the previous page and select "I will be providing my pets own food". 
  • Owner Supplied Pet Food

    If Plympton Vet Clinic is providing the food, please ignore this page
    • You must provide enough food for the entire duration of your pets stay
    • Plympton Vet Clinic does not have the ability to cook or warm food
    • Food provided by you must all be clearly labelled with your pets first and last name and be provided in clean and suitable containers or original packaging
  • Personal Belongings

    Bedding as well as food and water bowls are provided but you may wish to bring some of your own items. All pet owners are to acknowledge the following even if you plan not to bring any personal belongings.
  • Payment & Accounts

    Plympton Vet Clinic operates on a strict NO CREDIT basis - all accounts must be paid in full at the time of booking to secure the place in boarding.
  • Terms & Conditions

    • You confirm you have provided a copy of your pets most up to date vaccination records and medical history.
    • You confirm you have read, agreed and accept the terms and conditions notified in this booking agreement. 
    • In the event your pet required additional veterinary care during their stay, Plympton Vet Clinic will endeavor to contact you or your alternate contact for informed consent. If you or your alternate contact cannot be reached, you understand the clinic will perform any necessary treatment and that you are financially liable for any additional costs incurred.

    At Plympton Vet Clinic, we understand that sometimes boarding can be a new experience for your pet. A new environment and being away from loved ones can be stressful for some pets. Our goal is to provide a safe and comfortable stay. We prioritise your pets well-being and have some information on the best ways to prepare your pet for a stress-free stay.
    • Supporting Digestive Health 
    • A change of routine and environment can some times cause gastrointestinal upsets in pets.

      Routine is Key: If your pet has a sensitive stomach or is prone to stress/anxiety, please consider providing your own usual food to minimise digestive upset.

      Probiotics & Prebiotics: In the event a pet experiences gastrointestinal upsets, we offer a pre-approved gut support package. This includes both probiotic and prebiotic supplements and dietary modifications (such as switching to a bland diet or specific senstive food).

      Gut preparation: For enhanced support and a proactive approach to your pets gastrointestinal health, we recommend starting a probiotic/prebitoic 1 week prior to boarding and continuing this throughout their stay. We use Synbiotic D-C which can help maintain a balance gut microbiome and support immune function. For more information please visit Protexin: Synbiotic D-C

       

      If you would like to start a prebiotic/probiotic prior to your pets stay, please call the Plympton Vet Clinic to arrange dispensing the medication. 

    • Keeping Calm & Comfortable

      Vaccinations & Parasite Prevention: It is a requirement that your pets vaccinations are up to date. We strongly encourage your pets fleas, worming and parasite prevention is also up to date prior to boarding. 

      Health Conditions: Please ensure you correctly inform us of any pre-existing medical contions so we can ensure we are providing a safe and comfortable stay for your pet.

      Veterinary Care: We provide attentative care to all pets at Plympton Vet Clinic but unexpected health issues can arise. Please ensure you or your alternate contact are contactable at all times whilst your pet is boarding with us. 

      Pre-approval: Please consider providing pre-approval for medical treatment with our staff prior to your pets stay. This may include pre-approving a financial limit or specific medications should the need arise which would minimise delay in your pets treatment.

    • Familiar Items: Consider bringing a favourite blanket or toy to help your pet feel at home (Please ensure these are all very clearly labelled with your pets first and last name)

      Anxiety Management: If your pet has known anxiety (even if it does not normally require medications) please let us know.  We can organise a pre-boarding consultation with a vet to discuss options for their stay such as calming supplements, case specific training or medications if required.

  • Agreement

    Upon signing this form, you are hereby giving permission for Plympton Vet Clinic to board your pet for the duration of the booking. This form will be signed when you drop your pet off at PVC.
  • I, the undersigned, confirm that I am the legal owner or authorised caretaker of the pet listed in this form. I have read, understood, and agree to abide by the Plympton Vet Clinic Boarding Terms and Conditions provided to me. I acknowledge that my pet(s) will be cared for to the best of the facility’s ability, and I accept all risks associated with boarding, including unforeseen illness, injury, or escape. By signing below, I agree to all terms and conditions and understand this constitutes a legally binding agreement.
