1. Penulisan Nama yang Akurat

Mohon pastikan nama yang diisi pada formulir sudah benar dan tidak ada kesalahan penulisan (typo), karena nama tersebut akan dicetak di sertifikat. Tidak ada penggantian atau revisi untuk kesalahan yang disebabkan oleh pengisi formulir.

2. Divisi Paduan Suara

Harap lengkapi bagian divisi dengan jabatan Anda dalam paduan suara (misalnya: konduktor, penyanyi, musisi, atau official). Informasi ini penting untuk penerbitan sertifikat yang sesuai.

3. Total Biaya

Total biaya yang tertera adalah biaya pemesanan sertifikat di luar ongkos kirim. Ongkos kirim dari Bandung ke lokasi Anda akan dihitung secara terpisah.

The total amount displayed is the cost of the certificate order excluding shipping fees. Please note that shipping will be calculated separately from Bandung to your location.
