    • The JHA is a collaborative team effort. All workers shall participate in the completion of this form andsign prior to commencing work.
    • The JHA is a working document and therefore should be updated throughout the day if there are anychanges to risk, controls, hazards etc.
    • The JHA shall be reviewed and signed by any new crew member or visitor who arrives at site.
    • All incidents must be reported immediately to the Supervisor.
    • Hazards to be considered include: people, equipment, materials, environment, process, chemical,biological, physical, and ergonomic

    ** If gas is escaping: Shut off vehicles, equipment, and ignition sources, barricade the area off and keep workers and public away. Call 911 and Gas Utility immediately. Do not attempt to control escaping gas.
