I approve Cindy’s Twisted Stitches (CTS) to perform quilting services, and provide additional services, as described above.  I give permission for photographs of the quilt to be placed on the CTS website and used for promotional purposes. If this quilt is entered in any shows, I will credit Cindy Fox of Cindy’s Twisted Stitches for the quilting and notify her of any awards received.
 
Cindy’s Twisted Stitches makes every effort to treat your quilt top, shirts, and fabric with the utmost care and respect.  However, I acknowledge that unforeseen circumstances can occur such as fire, water damage, machine failure, etc. that could result in damage to the fabric/shirts/quilt top left in the care of CTS, and I agree that CTS is not liable for any damage or destruction of this quilt or t-shirts left on the premises.
